What is reflux?

Acid reflux occurs when stomach acid flows back into the oesophagus due to weakness or dysfunction of the valve between the oesophagus and stomach. This can cause symptoms such as heartburn, regurgitation, chest discomfort, and difficulty swallowing, and may lead to inflammation of the oesophagus if symptoms are persistent or untreated.

How is it investigated?

We receive referrals for patients with heartburn and reflux across Torquay and South Devon. We offer thorough investigation of acid reflux through gastroscopy which assesses the upper gastrointestinal tract with a specialised camera either with local anaesthetic spray or using light sedation.

Following on from this initial investigation and depending on findings, we are also able to perform specialised tests measuring acid reflux over a 24 hour period as well as assessing the motility of the oesophagus. Mr Kostalas undertakes and personally reports these investigations through a dedicated GI physiology service. These are gold standard tests that are key for any patient considering surgery as they allow us to specifically tailor treatments to the individual patient.

Gastroscopy can be performed at Mount Stuart Hospital, Torquay. All GI physiology investigations are performed at the GI physiology laboratory in Torbay Hospital.

How is it treated?

Reflux and heartburn treatment is multi-factorial. The first things to focus on our lifestyle adjustable factors and these can be discussed at consultation with Mr Kostalas. Once these factors have been optimised we look at the medical management of your symptoms. Medications that are often used are called proton pump inhibitors or hydrogen receptor antagonists. Reflux can often be transient and settle with a course of treatment.

Indications to proceed to surgical management of reflux are if a patient is unable to tolerate the medications that are prescribed to control reflux. Other reasons also include patient choice. There are many young patients with symptoms of reflux that would prefer definitive management for their symptoms rather than taking lifelong medication.

Finally surgery can be offered to those with symptoms that are persistent or not settling with medical management. The aim of surgery is to strengthen the mechanism at the bottom of the oesophagus that has weakend and is allowing acid contents to come up into the oesophagus from the stomach. There are different types of anti-reflux surgery all of which adopt a minimally invasive approach. The type of surgery you will be offered is based on the results of your investigations.
